Make a real impact on patient care as a Pharmacist specializing in Medication Therapy Management (MTM) with a leading healthcare provider working with Aquent. You will play a crucial role in improving patient health outcomes by providing comprehensive medication reviews and consultations. This is an opportunity to leverage your clinical expertise to make a tangible difference in patients’ lives.

As a Pharmacist focused on MTM, you’ll be responsible for conducting comprehensive medication reviews (CMRs) and providing in-depth consultations on statin medication use, adhering to American Heart Association (AHA) guidelines. You will engage with patients through outbound calls, gathering and transcribing essential prescription and diagnosis information into a secure web-based application. Your meticulous documentation will contribute to the creation of personalized patient documents. This role requires a high volume of patient interaction, with an expected 100 outbound calls per day.
What You’ll Do:

• Conduct outbound calls to patients (approximately 100 per day)
• Perform comprehensive medication reviews (CMRs)
• Deliver medication therapy management (MTM) services, including consultations on statin use per AHA guidelines
• Accurately and efficiently transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party web-based application
• Participate in training sessions to enhance skills related to essential job responsibilities

Must-Have Qualifications:

• Valid Pharmacist License in Florida 
• At least 4 years of experience as a clinical MTM pharmacist
• Familiarity with driving Part D Star measures, specifically SUPD, SPC, and CMR
• Strong understanding of pharmaceutical and medical terminology
• Clinical experience in medication review
• Fluency in spoken and written English
• Excellent verbal and written communication skills
• Tech-savvy and comfortable using web-based applications

Nice-to-Have Qualifications:

• Management experience
• Experience using Salesforce

The target hiring compensation range for this role is $53.87 to $59.86 an hour. Compensation is based on several factors including, but not limited to education, relevant work experience, relevant certifications, and location.
